Petition to Declare the Education and Applied Psychology Minor

This is required paperwork for every minor.

The “Petition to Declare the Minor” must be turned into the Gevirtz Graduate School (GGSE) Students Affairs office (Education 4100) in order to be awarded the Education and Applied Psychology Minor.

Substitution Form for the Education and Applied Psychology Minor

This is required paperwork if the student plans to substitute, waive, or enroll in courses outside of the prescribed track.
 

It is the student’s responsibility to contact the GGSE Student Affairs Office within the first 2 weeks in the quarter in which the degree is anticipated to certify fulfillment of degree requirements for the Office of the Registrar.

This is required paperwork for every Science and Mathematics Education minor. The “Petition to Declare the Minor” must be turned into the Gevirtz Graduate School (GGSE) Students Affairs office (Education 4100) in order to be awarded the Minor in Science and Mathematics Education.”
