This form is also available from the Education Program Office, ED 3102. Both student and instructor signatures are required. Completed forms should be returned to the Education Program Office, the approval code for GOLD registration will be emailed to you.

*ED 597, ED 598 or ED 599: Grad students need only the only the Instructor's approval to enroll. Ask your Instructor to email "progoffice@education.ucsb.edu" with their permission and an approval code to enroll on GOLD will be emailed to you.

ED 199 and ED 199RA

*Effective with W13 quarter: These courses do NOT require a form for enrollment.

Students may enroll with the Instructor's consent. Once approved, your Instructor will request that an approval code be emailed to you for enrollment on GOLD.

Instructors may send their email to: "progoffice@education.ucsb.edu" and an approval code will be issued to the student the same day.

*Undergrad Prerequisites for ED 199 and ED 199RA: Undergrad students must meet all prerequisites as listed in UCSB's General Catalog. Students are limited to 5 units per quarter and 30 units total in all Independent Study courses combined.

RESEARCH APPRENTICESHIP FORM

Students are expected to negotiate placement in a research apprenticeship with a faculty member during their first year of their enrollment in the emphasis. This requirement can be fulfilled through work as a research assistant, through a joint project with a faculty member, or through a variety of other arrangements. Upon completion of this apprenticeship, you should insure that the “Completion of the Research Apprenticeship” form (pdf) is signed by the supervising faculty member and filed in the Education Program Office.

INDEPENDENT RESEARCH PROJECT FORM

Completion of the project should result in a paper that is appropriate for submission for publication. In the writing of the paper, unless otherwise agreed to by the faculty committee, you should adhere in all matters of style to the Publication Manual of the American Psychological Association (latest edition).

Upon completion of the project, you should prepare the “Completion of the Independent Research Project” form (pdf). This form documents that you have fulfilled the Independent Research Project milestone requirement as well as the completion of the research project requirement for the Master's, if this option is available in your emphasis. Two faculty signatures are required for the milestone requirement and three are required for the Master's degree. The form must be filed in the Education Program Office.

DISSERTATION PROPOSAL APPROVAL FORM

After giving them at least a week to read the proposal, meet with your dissertation committee to review it. You will be expected to give an oral presentation outlining the rationale, purpose, and method of the project. As a result of this meeting the committee will decide if the proposal may be approved in its current form or if changes must be made. When the proposal is accepted, the committee must sign the "Dissertation Proposal Approval" form (pdf) and it must be filed in the Student Affairs Office.

For a course substitution the instructor of the new course must sign as well. Completed froms should be returned to ED 4109 in the Student Affairs Office.
